I understand that this membership requires a minimum commitment of 6 months.*

I agree to book my facial within 6 weeks of each visit to maintain active status.*

I understand that if I don’t book a facial within 6 weeks of my last visit, my membership will be paused and a $149 reactivation fee will apply.*

I understand that my membership will automatically renew month-to-month after the 6-month term unless canceled in writing.*

I understand I may pause my membership for up to 3 months after completing the 6-month term.*

I understand that cancellation requires a 30-day notice after the 6-month commitment, and that an early cancellation before completing the 6-month term will result in a one-time $149 fee.*

I understand that my membership benefits begin only after booking my first facial.*

I understand that the 15% facial discount, 10% skincare product discount, and 10% lash lift + tint discount apply only while my membership is active.*

I understand that member discounts cannot be combined with other offers or promotions.*

I understand that payment will be processed at each appointment, and that my credit card will be securely collected during my first visit and kept on file for membership billing.*
